We have an in-house Substance Use Counsellor!

• Help for people concerned with their own substance use, as well as someone else’s substance use
• Individual counselling sessions (assessment, referral, ongoing solution-focused therapy)
• Identify your goals
• Create a treatment plan using both abstinence and harm reduction approaches
• Education (available to both patients and their family members/loved ones)
• Individual and group counselling services offered in partnership with Homewood Community Addiction Services (CADS)
• Referral to other CADS support groups and programming in Guelph
• Referral to residential or day-treatment programs
• Connection with supports in local communities outside the Guelph region to better suit individual needs

Referrals

• Patients of the East Wellington Family Health Team can be referred directly by their family doctor
• Non-patients should contact HERE 24/7 (1-844-437-3247) to complete a referral
